Playing king of Castille was pretty rough.

I've had a lot of success starting in Ireland. You're fairly isolated and you have lots of other small duchies you can try and conquer, eventually building up to forming the kingdom of Ireland.

Ruler Designer
Legacy of Rome

I greatly enjoy starting with a customized ruler, and Legacy of Rome adds nice features to the game.

If you want the option to play as a Muslim leader/start in a Muslim land:
Sword of Islam

The rest have mainly aesthetic value, as far as I know.

In case anyone looking up to pick up the game + all expansions on the cheap, the entire CK2 collection plus the original CK is available for $10 on Amazon.

That price is insane. It even includes the latest expansion, The Republic.
